SCHEDULE Monday-Friday 8am-5pm

ESSENTIAL FUNCTIONS:

  1. Provide comprehensive clinical and administrative support for the Comprehensive Case Management Services (CCMS) program.
  2. Ensure high-quality, compliant, and client centered case management services that promote engagement, retention, continuity of care, and positive outcomes for individuals receiving or awaiting residential substance use treatment.
  3. Ensure CCMS services are trauma-informed, culturally and linguistically appropriate, and responsive to priority populations identified by HHSC.
  4. Support and guide case management activities, including completions of assessments, development of individualized service plans, facilitation of referrals, and maintenance or required client contact based on individual needs.
  5. Ensure services assist clients in accessing coordinated medical, behavioral health, housing, transportation, employment, education, and recovery support resources.
  6. Promote client engagement and continuity of care from waitlist through discharge and required follow-up periods.
  7. Work with other CCMS case managers to support service quality and compliance.
  8. Ensure accurate, timely, and complete documentation of all CCMS services, contacts, referrals, and follow-ups in the EHR system.
  9. Monitor EHR records for compliance with HHSC standards, including case opening and closure, service plans, progress notes, referrals, and discharge follow-ups.
  10. Compile, review, and submit required monthly performance metrics in EHR ensuring accuracy and alignment with HHSC performance and outcome measures.
  11. Prepare and submit Quarterly Activity Reports documenting program accomplishments, barriers to care, gaps in community resources, outreach to underserved populations, and progress toward grant goals.
  12. Monitor CCMS performance trends, service volume, and outcomes to support continuous quality improvement efforts.
  13. Collaborate with program and organizational leadership on HHSC data verification or monitoring requests.
  14. Participate in quality management, monitoring, and continuous quality improvement (CQI) activities as required by HHSC.
  15. Assist with preparation for HHSC monitoring reviews, audits, and site visits.
  16. Support the implementation and ongoing maintenance of the program's Quality Management Plan (QMP).
  17. Ensure client satisfaction surveys are conducted, documented, and submitted in accordance with HHSC timelines.
  18. Maintain ongoing compliance with all required training courses relevant to the role, including but not limited to company-mandated courses and any state-regulated certifications or training requirements.
  19. Ensure completion of Big 5 performance management tool for staff by the required deadlines and use of SMART goals to support annual performance compensation review.
  20. Work closely with management team to uphold the core principles of Cenikor's culture and create a positive working environment for all team members.
  21. Perform additional duties as assigned and consistent with the non-exempt functions as assigned in this job description.

QUALIFICATIONS FOR THE POSITION:

Skills, Knowledge and Abilities:

  • Possesses good oral and written communication skills.
  • Experienced and/or knowledgeable in providing education and therapeutic counseling for individuals and for groups.
  • Experienced and capable of identifying appropriate resources for client development and providing substance use counseling.
  • Capable of discerning facts from opinions and remaining objective when listening to conflicting opinions.
  • Experienced in working with diverse population.
  • Possesses good time management skills needed to meet aggressive deadlines and prioritize multiple tasks.
  • Patience, perseverance, and resilience in a challenging environment.
  • Knowledge of Microsoft Office software required, including specific MS Outlook calendar experience.

Education:

  • High school diploma or GED required

Experience:

  • Two (2) years of substance use treatment experience preferred.
  • Experience with CMBHS or other electronic health record system preferred.

Physical Requirements:

  • Standing and/or sitting for extended periods of time.
  • Reaching by extending hand(s) or arm(s) in any direction.
  • Finger dexterity required to manipulate objects with fingers rather than with whole

hand(s) or arm(s), for example, using a keyboard.

  • Communication skills using the written and spoken word.
  • Ability to see within normal parameters.
  • Ability to hear within normal range.
  • Possible exposure to communicable diseases.
  • Participation in activities outside of the facility or traditional business hours is required.
  • Travel required for out of town work related activities.
